Continuum Bridge Care Homes is an independent living with care-management services based in Gardena, California. We serve residents transitioning from hospitals, rehabilitation facilities, or unstable housing by providing safe housing, care coordination, and supportive living solutions.
We begin with a comprehensive assessment to understand the resident's medical, social, and personal needs. Based on this, we develop a personalized care plan, coordinate the transition with all involved parties, and provide ongoing support to ensure a safe, seamless move-in experience with continuity of care as main priority.
We can typically facilitate move-in within 24–72 hours, provided all required documentation has been received. Timing may vary based on the individual's care needs and availability. We work closely with discharge planners and families to ensure a smooth and timely transition.
Yes. We work with Medi-Cal members and collaborate with Enhanced Care Management (ECM) teams to help coordinate housing and supportive services. When appropriate, we can connect residents with trusted third-party providers for IHSS caregivers, Home Health services (including physical, occupational, and speech therapy), and other community resources. We also provide complimentary care coordination to help families navigate the transition and ongoing support services.
No. As a non-medical independent living home, we do not provide caregivers or personal care services. However, we can help coordinate caregiver support through trusted third-party providers, including IHSS, Home Health, and other community-based services when appropriate.
