Continuum Bridge Care Homes provides structured, non-medical housing for adults transitioning from hospitals, skilled nursing facilities, rehabilitation programs, correctional reentry, and other community-based settings.

Our mission is simple: provide a safe home, restore stability, and help every resident move toward greater independence.

We are not a hospital, nursing home, assisted living facility, or medical provider. Instead, we provide a supportive home environment while working alongside healthcare professionals, families, and community partners to coordinate the services residents need to succeed.

Whether someone is recovering after a hospitalization, rebuilding after a difficult life event, or simply needs a stable place to regain independence, our homes provide the structure, dignity, and support to help them move forward.
